A complete application includes each of the items below. After reviewing the application we will let you know whether you are invited for the next stage of the application process, which will include interviews.

Checklist of Application Materials

Please submit the following documents on line:
o    J-CHAI Application for Admission - Parts 1, 2 & 3
o    Applicant Questionnaire
o    Parent/Guardian Questionnaire
o    Release Authorization Form
o    Three letters of recommendation from non-relatives who have known you for at least one year.   These can be filled out either on line or  distributing recommendation forms, please provide a stamped envelope addressed to J-CHAI.  
o    $ 150 non-refundable application fee.  Online payment (coming soon) or by check or money order payable to “Etta Israel Center” and mail to J-CHAI.

Please mail the following documents

o    Most recent assessment that might include a cognitive evaluation and/or academic assessment, preferably not more than 3 years old
o    Your exit IEP from high school and a most recent IPP from Regional Center, if applicable  
o    Transcripts from all secondary and post-secondary educational institutions, if applicable
o    Regional Center files released and sent to J-CHAI, if applicable
o    A recent photo/head shot
